All assistance provided will be made payable to the appropriate vendor, not the household. Rental assistance in excess of $600 requires that the landlord provide a W-9 form.
Rent and/or utility assistance will be determined on a case-by-case basis depending on the total loss of household income directly due to the COVID-19 pandemic and off-set by any replacement of income from unemployment, disability, paid family leave, sick or vacation leave, or other direct assistance due to this crisis.
The amount of assistance will be based in part on household income directed to critical housing costs prior.
The current ratio of essential housing costs to income must exceed 30% .
Eligible households may receive assistance as long as there is a demonstrated need and as long as program funding is available. Assistance for each month will requires re-evaluation of need. Some localities limit assistance to three months.
Resident of the locality providing funding prior to February 1, 2020.
Current income cannot exceed 80% of area median income (AMI) based on household size.

Past due rent caused by the impact of COVID-19.
Past due rent prior to March 13, 2020 are not eligible for assistance under this program.
Basic information on all household members (date of birth, gender, etc.).
Lease/rental agreement identifying the members of the household and the normal rent amount.
Documentation of all household income prior to and after qualifying event, demonstrating income loss.
General household budget showing normal income and expenses.
Applicants denied assistance will be provided with a written reason for the denial. Applicants who believe they have been denied unfairly have the right to file a grievance. Click here to file a grievance.
